What Community Health Worker Do During Home Visit

A home visit is one of the activities under the community health work programs aimed at supporting community members by providing an opportunity for them to discuss problems around family and community are facing and finding solutions to keep family and community healthy. This protocol provides a guideline how to meet, provide a good environment, positively converse, ask simple questions, record, negotiate, deal with issues and plan activities to help their community. When conducting home visit, community health worker are required to complete report forms to be signed by clients for reporting the activity and to act as a reminder for any follow-up activities.

The objective of this is to build closer relationship with family member, community members. Through this home visit, Community Health Worker and Health personnel can provide them with the health-related information or primary health care information, talk about the progress of health in the their community and providing feedback to them and link community members to the existing resources or services in their communities.

Establishing good relationship family and community members can be achieved through using basic skills in good communication and attitude of the home visitors. The significance of the home visit to is to increase community members’ involvement in encouraging and supporting their family hygiene and health education and also so that home visitors have the opportunity to raise any issues and provide encouragement to keep family and community healthy. Major items of successful and effective home visit:

1. Communication skills and facilitation skill such as suitable time, listening, identification logical responses, and steps of problem-solving.
2. Understanding, compassion when someone shows you a problem.
3. Being aware and responding to body language: facial appearance, body language and expression of attention.
4. Listening and questioning skills.
A home visit activity should follow three steps, (1) Preparation before visit, (2) Activities during home visit, and (3) Activities following home visit. And these steps include vital items: plans, forms to be used, target and methods of face-to-face conversation and problem-solving. However, in general it should take no less than one hour, excluding waiting time and other arrangement.

Preparation Ahead of Home Visit

1. Think of purpose and plan for home visit (roles of the visitors)
2. Provide all stakeholders again with information on the venue and time
3. Check you have all the required documents or forms and information on this student
4. Make sure of the venue and direction to go (if it’s a home visit)
5. Contact all appropriate stakeholders on this activity
Activities During Home Visit

1. We should properly stand before starting a debate “Hello! How are you doing? Would you mind us asking you some questions?”
2. Use positive words related to the family or environment around the house.
3. Introduce yourself to the house owner.
4. Tell them about the purpose of home visit and make sure it is convenient for them to discuss their issues around family such health of family members or other issues even if you have already arranged this appointment

5. Find a suitable place where you can sit and discuss together privately and where they feel comfortable.
6. When introducing the problem their children’ health facing, notify them about positive points.
7. Brainstorm and discuss with them to identify issues and problem-solving.
8. Finally, please ask them if they have any questions; thereafter, say a big thank-you to everyone who has taken time discussing the issues facing in the family.

Activities Following Home Visit: Review pledges and agreements which have been cooperatively decided during the discussion arrange reports and review the action plan to be implemented.

Therefore, conducting a home visit by Community Health Worker in community can reach any particular purpose unless we can identify and assess factors affecting during such as sharing information with all stakeholders and limiting appropriate and usable resources.
